What are your clinic's operating hours?

Our clinic is open Monday to Friday, from 8:00 AM to 5:00 PM. We are closed on weekends and major holidays.

We have multiple convenient locations. Please visit our ‘Contact Us’ page or use the ‘Find a Location’ tool for addresses and directions.

While some insurance plans may require a referral, many do not. We recommend checking with your insurance provider or contacting our office for clarification.

Please bring your insurance card, photo ID, a list of current medications, and any relevant medical records or imaging reports.

Yes, ample free parking is available directly adjacent to all our clinic locations for your convenience.

For prescription refills, please contact your pharmacy directly. They will send a request to our office for approval.

Appointments & Billing

How do I schedule or reschedule an appointment?

You can schedule or reschedule by calling our main office line at 833 783 6561 during business hours or through your Patient Portal account.

We kindly request at least 24 hours’ notice for cancellations or rescheduling. This allows us to offer the slot to another patient.

We work with a wide range of insurance providers. Please see our Accepted Insurance Plans page or call our billing department to verify your coverage.

We accept cash, major credit cards (Visa, Mastercard, American Express, Discover), and offer flexible payment plans for eligible patients.

You can pay your bill securely online via the Patient Portal, by mail, or in person at our front desk during clinic hours.

These are terms related to your insurance plan. A co-pay is a fixed fee, a deductible is the amount you pay before insurance starts to cover costs, and co-insurance is a percentage of costs you pay after meeting your deductible.
